REGISTRATION

Prior to any participation in the CMD Program, prospective divers and parents must complete the following forms:

CMD forms with HC Wavier (downloadable below) but can use this

JOIN GOOGLE FORM in leau of paper. Quicker registration.

And finally would need AAU MEMBERSHIP FOLLOW DIRECTIONS BELOW

then follow these steps:

1) Click on Member login on top right corner

2) Create an account if you don't have one yet
3) Login and then purchase an Athlete membership

ATHLETE MEMBERSHIP::
Step 1) WHAT TYPE OF MEMBERSHIP:
       -Program Type: Youth Program
       -Sport: Diving (DI)
        -Membership Category:

Athlete membership -> 1 yr $20

Step 2) MEMBER DETAILS (GENERAL INFORMATION)
Step 3) ARE YOU A MEMBER OF A CLUB?

- SELECT "YES"

-Then Select "CENTRAL MASS DIVING" or enter club code: " W3DF46"
Continue with the application and then click “submit this application”. Once that is completed you are eligible to dive.

* Just add your new AAU membership # to CMD registration form.

When you turn in the form you can return to me via email: wil@centralmassdiving.com or on deck. I only need pages 1,2,3,7 & 8 returned as pages 4,5,6 acts as your reference.
